Waivers, Consents Releases & Notices

I am one of the parents or other legal guardians of the child who is registering and enrolling in Neuron Garage. I represent that I am authorized to sign this document on behalf of all parents and any other legal guardians of the child. In consideration for my child, as identified above, being allowed to participate fully in all Neuron Garage activities, I hereby:

 

  1.   waive, release, and discharge any and all claims for damages for personal injury or property damage which may hereafter occur to him or her as a result of participation in Neuron Garage activities. This waiver, release, and discharge are intended to discharge in advance Impact Endeavors, LLC doing business as Neuron Garage, its owners, officers, employees, volunteers and agents from liability, even if that liability may arise out of negligence on the part of persons mentioned above. I understand that some activities may involve an element of risk or danger of accidents, and knowing those risks, I assume them on behalf of my child. I further agree that this waiver, release, discharge and assumption of risk is to be binding on my spouse or partner, any other parent, my heirs and assignees, and any other persons who may seek to claim damages as a result of any injury to my child or other damage. Furthermore, I agree to indemnify Impact Endeavors, LLC from any claims resulting from participation in the camp, including payment of reasonable defense costs incurred by Neuron Garage. If any aspect of this waiver, release and discharge is deemed to be invalid, I acknowledge that the remainder of the agreement will continue to have full force and effect.

 

  1.   I give permission to Neuron Garage staff to administer any provided medication, provide routine first-aid and to seek emergency services. I further give my consent to have my child treated by emergency medical personnel, a physician, or surgeon, in case of sudden illness or injury while participating in Neuron Garage activities, and being transported in order to obtain that treatment. I understand that Neuron Garage will not provide medical insurance for such treatment, and that the cost thereof will be at my expense.

 

  1.   I give permission to release my child/ren from Neuron Garage to any parent/ guardian or persons listed under Authorized Pick-ups.

 

  1.   I understand that my child’s participation in this camp includes building with recyclable materials, tape, scissors, low-temp hot glue guns, and other simple tools and materials, including some that are sharp. In addition, during breaks from building, my child will participate in outdoor and indoor play, including but not limited to, access to playscapes, fields, and concrete play areas, which may or may not be fenced in. All of the aforementioned activities carry some physical risk.

 

  1.   I understand that Neuron Garage offers pre-packaged snacks to Campers. A sample of these is available at the Check-In table and I am expected to review the ingredients in the pre-packaged snacks and speak with my child and location Director about any off-limit foods. While our pre-packaged snacks typically do not contain nuts, we are not a nut-free campus and cannot guarantee that the food other Campers bring to camp will be nut or other allergen-free.

 

  1.   I understand that Neuron Garage is not licensed by the State of Texas or Colorado. However, all staff are CPR/First Aid certified and have completed background checks.

 

  1.   I understand that Neuron Garage utilizes various facilities to operate the program and each facility is different and will carry its own risks.

 

  1.   I understand that photos and/or videos may be taken of my child in various activities while at camp. No last names will accompany these images or videos, but they may be used without compensation for promotional or training purposes. Additionally, photos and videos from each week may be shared so that families can get a glimpse into the Neuron Garage experience.

 

  1.   I understand that we may ask Campers who require 1:1 supervision, disrupt the camp experience, or threaten the safety of themselves or others to leave camp. All participants are expected to conduct themselves in a responsible, respectful, and safe manner. Neuron Garage strives to create a fun, safe environment in which each child thrives individually and as a member of the community. Neuron Garage staff members are trained to use positive reinforcement, modeling, logical consequences, and discussion to guide behavior. If there is a situation in which these methods are not effective, you will be contacted to discuss additional intervention, up to and including, removal from the program.
